Therapy Assistant (OTA) Admission Procedures CRITERION FOR ACCEPTANCE - ACCEPTANCE INTO THE OTA PROGRAM IS ON A BEST-QUALIFIED, FIRST-ACCEPTED BASIS. Qualification is based on a rating system for (1) grades for completed prerequisite courses and (2) completion of occupational therapy field observation/shadowing (8 hours minimum). Selection is based on total qualifying scores in rank order from the highest until admission quota is reached.

II. Submit a “University
of Hawai’i System Application Form” if you are: Eligibility: All persons who are 18 or older, or who have earned a high school diploma or equivalency, are eligible for admissions to Kapi’olani Community College. Identify your major as: “Liberal Arts (Pre-OTA).” The College requires tuberculosis (TB) and Measles Mumps Rubella (MMR) clearance for registration approval. Application form is available in the Kekaulike Information and Service Center (KISC), Ilima 102. Information and the application form are also available on-line. STEP
III. On Walk-in Wednesdays, you may seek advising to develop an academic action plan with a Health Sciences counselor using a “student issued copy” of your transcripts/records. STEP
IV. Complete all OTA prerequisite courses prior to application. A “C” grade or higher is required for prerequisite and support courses (See attached OTA curriculum). Science courses need to be completed within the last 5 years to be valid. Completing other program support courses is highly recommended. To register online, go to http://myuhportal.hawaii.edu and follow instructions. STEP
V. For evaluation of your previous college work to be applied toward your degree, you should have official transcript(s) and course description(s) outside Kapi'olani Community College be sent directly to the Kekaulike Information and Service Center (KISC). You must complete a “Request for Transcript Evaluation” form. STEP
VI. Pick up the OTA Application
Packet in Kauila 122. The application packet is available in the “kiosk” during
Health Sciences office hours from 8:00-11:30 a.m. and 1:30-4:30 p.m.

Monday through Friday, except state holidays. The application packet
is not available on-line. Neighbor island and U.S. mainland students
may request the application packet be sent by mail from the Health Sciences
department. The OTA Program application packet includes the (a) OTA Application Checklist (b) Application for Selective Admission Program (ASAP form) (c) OTA Admission Information form. NOTICE: "Health care students are required to complete University prescribed academic requirements that involve clinical practice in a University affiliated health care facility setting with no substitution allowable for the completion of required clinical practice. Failure of a student to complete the prescribed clinical practice shall be deemed as not satisfying academic program requirements. It is the responsibility of the student to satisfactorily complete any background checks and drug testing that may be required by the affliated health care facility to which he/she is assigned for clinical practice in accordance with procedures and timelines as prescribed by that affiliated health card facility." STEP
VII. Each applicant will receive a letter of application status by the end of June. Accepted students will receive an Acceptance Letter from the Program Director about the Orientation and Advising Sessions. Students who are not selected are encouraged to seek support for future acceptance from Program Director or Health Sciences counselor. You may apply for financial assistance at the Financial Aid Office. STEP
VIII. All new OTA majors will attend a Program Orientation and Advising session. At orientation, students will review the OTA Program Student Handbook. During the advising session, OTA majors will be advised by the Program Director about the OTA courses to enroll in for the Fall semester. Pay for your liability insurance at the Business Office (Ilima 101). Books may be purchased at the KCC bookstore the same day. Complete and return your health forms and First Aid & CPR certification to the Program Director by the first week of the Fall semester. STEP
IX. You will register for your OTA courses on-line. To register online, go to http://myuhportal.hawaii.edu and follow the instructions to login. Payment may be made on-line or at the cashier’s office (Ilima 102). STEP
X. The KeyTrain Program equips students with work skills to prepare for job entry. New OTA majors will participate in a session for KeyTrain program. Students will complete the KeyTrain pre-assessments by the designated deadline. Students can begin and complete the KeyTrain “lessons” to improve their “skill gaps” of the job profile. OTA majors will complete the WorkKeys post-assessment near program exit. ADDITIONAL SOURCES OF INFORMATION CATALOG: You may review the Kapi'olani Community College General Catalog for general information about admissions and the current Occupational Therapy Assistant program curriculum and course descriptions. Additional information is listed under the heading "Special Requirements for Programs in Health Career Education." A Catalog and Schedule of Courses may be purchased at the KCC bookstore or viewed online. COUNSELOR: (NOTE: For all Health Sciences and EMS programs) For Academic Advising and Support, you may visit with a Health Sciences counselor on Walk-In Wednesdays in Kauila 122 or call 808.734.9270 to make an appointment or use the Ask-A-Counselor form for e-mail advising. Admission requirements and application procedures can change. Prior to applying to the OTA program, contact the Health Sciences Department for updated information.
